PROVIDER:

JULIE SIELING, F.N.P.-C., FAMILY MEDICINE

Julie Sieling serves as a primary care provider for patients ages 2 and up and is currently accepting new patients.

As a family nurse practitioner, Julie is an excellent resource for routine check-ups, preventive medicine, immunizations and physicals, illness and injuries, nutrition and lifestyle counseling, and health risk assessments for children, adolescents, adults and seniors.

Prior to becoming a nurse practitioner in 2010, Julie worked as a registered nurse in various roles in the hospital setting for 17 years. She received her master’s degree in nursing and Family Nurse Practitioner certificate from Rush University. She also has a bachelor’s degree in nursing from Olivet Nazarene University.

Additionally, she is board certifi­ed by the American Association of Nurse Practitioners and is also a certifi­ed Breast Patient Navigator.

PREPARING FOR YOUR APPOINTMENT

For your convenience, we suggest that you download and complete our Healthcare Centers – General Patient Registration Form before coming in for your office visit.

Please arrive 10-15 minutes prior to your appointment time to complete registration and a health questionnaire.

When you come for your appointments, please remember to bring the following:

  • Driver’s License or a valid ID
  • Insurance information
  • Referral Letter (if required)
  • Reports, X-rays, MRIs, CT scans, etc. and any other relevant information
  • List of medications (if any)

We recognize that your time is valuable, and we make every effort to stay on schedule. Occasionally emergencies or patients require a little more time and may cause delays beyond our control. We apologize if we keep you waiting.

TO CANCEL AN APPOINTMENT

If you need to cancel an appointment, please call the office during business hours at least 1 day prior to your scheduled appointment so that we can offer your appointment time to patients on our waiting list.
