Dr. Jennifer Thomas, Gardner Healthcare Center of Morris HospitalAs a family medicine physician, Dr. Jennifer Thomas provides care for the entire family, from newborns to seniors, including preventive care, routine check-ups, health risk assessments, immunizations, screening tests, and personalized counseling on maintaining a healthy lifestyle. She is committed to providing a friendly and accommodating office environment for her patients and believes in placing the needs of her patients first. Dr. Thomas encourages patients to have autonomy and a voice in all decision-making related to their care.

Dr. Thomas received her medical degree from Southern Illinois University School of Medicine in Springfield and completed a family medicine residency at Adventist Hinsdale Hospital. She also completed a primary care psychiatry fellowship through UC Davis in 2018 and is the Medical Director of Morris Hospital’s Integrated Behavioral Health program. She is board certified in family medicine and addiction medicine.

When she’s not taking care of patients, Dr. Thomas enjoys cooking, art, and reading. She is married to Mark Thomas, who teaches at Minooka Community High School.

KATHY ULIVI, F.N.P-B.C., FAMILY MEDICINE

As a family nurse practitioner, Kathy Ulivi is qualified to serve as the primary healthcare provider for patients of all ages. She encourages her patients to participate with her in developing their plan of care, encompassing all aspects of physical, emotional and spiritual well being.

Kathy received her Master’s of Nursing -Family Nurse Practitioner degree from Olivet Nazarene University. She also holds of Master’s of Nursing in Education from Lewis University. In addition, Kathy is a certified diabetic educator, an oncology certified nurse, and she is board certified in pain management. She also has an interest in integrated behavioral health.

Kathy enjoys spending time with family and friends, teaching, reading and volunteering.

PREPARING FOR YOUR APPOINTMENT

For your convenience, we suggest that you download and complete our Healthcare Centers – General Patient Registration Form before coming in for your office visit.

Please arrive 10-15 minutes prior to your appointment time to complete registration and a health questionnaire.

When you come for your appointments, please remember to bring the following:

  • Driver’s License or a valid ID
  • Insurance information
  • Referral Letter (if required)
  • Reports, X-rays, MRIs, CT scans, etc. and any other relevant information
  • List of medications (if any)

We recognize that your time is valuable, and we make every effort to stay on schedule. Occasionally emergencies or patients require a little more time and may cause delays beyond our control. We apologize if we keep you waiting.

TO CANCEL AN APPOINTMENT

If you need to cancel an appointment, please call the office during business hours at least 1 day prior to your scheduled appointment so that we can offer your appointment time to patients on our waiting list.
