Medical Records:
We have partnered with MediCopy to fulfill Release of Information requests. MediCopy is fully HIPAA compliant and adheres to all state and federal regulations concerning the release of medical information. Your medical record contains information regarding your personal health and the care you received at Morris Hospital or one of our affiliated locations.
Requesting Medical Records
Patients:
- Patients: To request your medical records please visit MediCopy.net/Patients. Please provide an email address on the authorization to expedite the process and delivery method. Records sent via email, or to another provider, are complimentary. Requests are processed within 7-10 business days after receipt by MediCopy.
- If you prefer to complete a paper authorization form, please download and print the “Authorization to Disclose Health Information.” You can access this form by clicking here to go to our Forms Center.
- Completed forms may be returned to your providers office, by mail or fax at 888-975-2304. Please follow the instructions as indicated on the form. Requests are processed within 7-10 business days after receipt by MediCopy. You may pick up your records or request that they be sent in the mail.

Fees
There are no fees associated with requests for information to be sent directly to another medical provider outside of Morris Hospital & Healthcare Centers. There is no fee to have medical records emailed to yourself. There may be fees associated for copies requests to be sent to you or to someone who is not a medical provider. If there is a fee for your request, MediCopy will provide you with an invoice summarizing the charges.

Images:
To request a copy of your images (x-ray, CT, MRI, ultrasound, mammogram), please call the Imaging department at 815-705-7130 Monday-Friday from 6:00 a.m.- 5:00 p.m. Images will be put on a compact disc (CD) and will be available for pick up in Medical Records in the Main Lobby Monday through Friday between the hours of 8:00 a.m. and 4:30 p.m. Please pick up within 30 days from the date of request. A photo-ID will be required at the time of pick up.
Birth Certificate:
For a copy of your original birth certificate, please contact the Grundy County Clerks Office
