In order to renew an IDPH license, pre-hospital providers are required to submit certain documentation. These requirements of what must be turned into the EMS System are listed below.
Items can either be hand delivered to the EMS System Office or they can be e-mailed to kgeiger@morrishospital.org.
Please have all documents turned into the EMS System Office at least 30 days prior to expiration of license to ensure that there is no lapse in licensure.
EMD
Required documentation:
- IDPH license
- Child Support/Felony Conviction Statement
- Receipt of Payment to IDPH
- AHA-CPR or Instructor card
- 12 Hours of CE every 4 years to renew
EMERGENCY MEDICAL RESPONDER (FIRST RESPONDER/DEFIBRILLATOR)
Required documentation:
- IDPH license
- Child Support/Felony Conviction Statement
- Receipt of Payment to IDPH
- AHA-CPR or Instructor card
- 24 hours of CE every 4 years to renew
EMT (EMT-BASIC)
Required documentation:
- IDPH License
- Child Support/Felony Conviction Statement
- Receipt of Payment to IDPH
- AHA-CPR provider or Instructor card
- PHTLS or ITLS recommended but not required
- 60 hours of CE every 4 years to renew
EMT-INTERMEDIATE
Required documentation:
- IDPH license
- Child Support/Felony Conviction Statement
- Receipt of Payment to IDPH
- AHA-CPR provider or Instructor card
- ACLS provider or Instructor card (If working for ILS/ALS Agency and/or your Primary in MHEMSS)
- PALS provider or Instructor card (If working for ILS/ALS Agency and/or your Primary in MHEMSS)
- PHTLS or ITLS recommended but not required
- 80 hours of CE every 4 years to renew
EMT-PARAMEDIC
Required documentation:
- IDPH License
- Child Support/Felony Conviction Statement
- Receipt of Payment to IDPH
- AHA-CPR provider or Instructor card
- ACLS provider or Instructor card (If working for ILS/ALS Agency and/or your Primary in MHEMSS)
- PALS provider or Instructor card (If working for ILS/ALS Agency and/or your Primary in MHEMSS)
- PHTLS or ITLS recommended but not required
- 100 hours of CE every 4 years to renew
PHRN
Required documentation:
- IDPH License
- Child Support/Felony Conviction Statement
- Receipt of Payment to IDPH
- AHA-CPR provider or Instructor card
- ACLS provider or Instructor card
- PALS provider or Instructor card
- PHTLS or ITLS recommended but not required
- 100 hours of CE every 4 years to renew
ECRN
Required documentation:
- IDPH License
- Child Support/Felony Conviction Statement
- Receipt of Payment to IDPH
- AHA-CPR provider or Instructor card
- ACLS provider or Instructor card
- PALS provider or Instructor card
- TNS, TNCC, ITLS or PHTLS Card
- 32 hours of CE every 4 years to renew
