Morris Hospital EMS System entry is scheduled through your department EMS System Coordinator and is done at the Morris Hospital EMS Training Academy located in the Old Shabbona Middle School in Morris IL (725 School Street, Morris IL 60450).
All requirements below must be turned in with a signature of your department coordinator and tests passed with at least a score of 80% to gain system entry. One remediation attempt will be allowed one week later if 80% is not achieved.
BLS WITHIN REGION 7
Requirements:
- Letter of Good Standing with CE hours (Optional)
- Proof of 2022 BLS SMO Update
- Demographic Sheet
- Copies of all licenses and CPR card
ALS WITHIN REGION 7
Requirements:
- Letter of Good Standing with CE hours
- Proof of 2022 ALS SMO Update
- Medical Math Exam
- EKG Exam
- Demographic Sheet
- Copies of all License’s and Certifications (CPR, ACLS, PALS)
BLS FROM OUTSIDE REGION 7
Requirements:
- Letter of Good Standing with CE hours
- BLS SMO Test
- Demographic Sheet
- Copies of all licenses and CPR card
ALS FROM OUTSIDE REGION 7
Requirements:
- Letter of Good Standing with CE hours
- ALS SMO test
- Medical Math test
- EKG Exam
- Demographic Sheet
- Copies of all license’s and Certifications (CPR, ACLS, PALS)
