image of Dr. Nuzhat ChalisaMedical conditions associated with the glands and hormones often require the expertise of a physician who specializes in the endocrine system. Dr. Nuzhat Chalisa provides diagnosis and treatment of diabetes, thyroid disorders, osteoporosis, obesity and pituitary problems.

Dr. Chalisa believes in the principle of “Act on Diabetes Now,” which provides a multi-systemic approach to the promotion of diabetes education as a core component of diabetes care. She is committed to providing excellence in the care of diabetes and other endocrine disorders.

Dr. Chalisa completed her residency at Loyola University Medical Center and a fellowship in endocrinology at Rosalind & Franklin University of Health and Sciences/Chicago Medical School. She is board certified in internal medicine and endocrinology.

A resident of Naperville, Dr. Chalisa is married and has two children. She enjoys traveling and interior decorating and is fluent in English, Urdu, Hindi, and Gujrati.

How do I prepare for my appointment?

Before your first visit, we suggest that you go to our Forms Center and download and complete our Healthcare Centers – General Patient Registration Form.

Please arrive 10-15 minutes prior to your appointment time to complete registration and a health questionnaire.

When you come for your appointments, please remember to bring the following:

  • Driver’s License or a valid ID
  • Insurance information
  • Referral Letter (if required)
  • Reports, X-rays, MRIs, CT scans, etc. and any other relevant information
  • List of medications (if any)

We recognize that your time is valuable, and we make every effort to stay on schedule. Occasionally emergencies or patients require a little more time and may cause delays beyond our control. We apologize if we keep you waiting.
