NUZHAT CHALISA, M.D., ENDOCRINOLOGY

Dr. Nuzhat Chalisa, endocrinologyMedical conditions associated with the glands and hormones often require the expertise of a physician who specializes in the endocrine system. Dr. Nuzhat Chalisa provides diagnosis and treatment of diabetes, thyroid disorders, osteoporosis, obesity and pituitary problems.

Dr. Chalisa believes in the principle of “Act on Diabetes Now,” which provides a multi-systemic approach to the promotion of diabetes education as a core component of diabetes care. She is committed to providing excellence in the care of diabetes and other endocrine disorders.

Dr. Chalisa completed her residency at Loyola University Medical Center and a fellowship in endocrinology at Rosalind & Franklin University of Health and Sciences/Chicago Medical School. She is board certified in internal medicine and endocrinology.

A resident of Naperville, Dr. Chalisa is married and has two children. She enjoys traveling and interior decorating and is fluent in English, Urdu, Hindi, and Gujrati.

JENNIFER GREGGAIN, N.P., ENDOCRINOLOGY

Jennifer Greggain, NP-BC, EndocrinologyJennifer Greggain is an endocrinology nurse practitioner serving under Dr. Nuzhat Chalisa. She is qualified to diagnosis and treat endocrine disorders, including diabetes, thyroid disorders, polycystic ovarian syndrome, osteoporosis, obesity and pituitary problems. She encourages patients to be actively involved in their healthcare decisions.

Jennifer received a Master’s of Nursing – Family Nurse Practitioner degree from the University of St. Francis in Joliet. She enjoys spending time with family, reading, gardening and staying physically active.

HOW DO I PREPARE FOR MY APPOINTMENT?

For your convenience, we suggest that you download and complete our Healthcare Centers – General Patient Registration Form before coming in for your office visit.

Please arrive 10-15 minutes prior to your appointment time to complete registration and a health questionnaire.

When you come for your appointments, please remember to bring the following:

  • Driver’s License or a valid ID
  • Insurance information
  • Referral Letter (if required)
  • Reports, X-rays, MRIs, CT scans, etc. and any other relevant information
  • List of medications (if any)

We recognize that your time is valuable, and we make every effort to stay on schedule. Occasionally emergencies or patients require a little more time and may cause delays beyond our control. We apologize if we keep you waiting.

HOW DO I CANCEL AN APPOINTMENT?

If you need to cancel an appointment, please call the office during business hours at least 1 day prior to your scheduled appointment so that we can offer your appointment time to patients on our waiting list.

HOW DO I ACCESS THE PATIENT PORTAL?

We are pleased to offer a patient portal that gives you access to all of your health information, test results, office visit summaries and appointments associated with Morris Hospital & Healthcare Centers. You can also use your portal to request medication refills, message your Morris Hospital Healthcare Center provider, and pay Morris Hospital bills. Please follow this link to My Health@Morris Hospital for more information, including how to enroll.

(If you have a Physician Office portal account (Healow app), please note that we made the transition to one patient portal on April 1, 2023. While your Physician Office portal will still be available for viewing for a limited time, no new information will be added to this portal beginning April 1.
