MEET DR. VERMILLION:

DAVID VERMILLION, M.D., INTERNAL MEDICINE, PEDIATRICS

Board-certified in internal medicine and pediatrics, Dr. Vermillion provides primary care for patients of all ages. He has been a member of the Morris Hospital medical staff since 1993.

Dr. Vermillion attended medical school at the University of Illinois, College of Medicine – Rockford and completed his residency at Loyola University Medical Center.

HOW DO I SCHEDULE AN APPOINTMENT?

To schedule an appointment or for more information about the Morris Healthcare Center of Morris Hospital at Division Street, please call our office at 815.513.5626.

HOW DO I PREPARE FOR MY APPOINTMENT?

For your convenience, we suggest that you download and complete our Healthcare Centers – General Patient Registration Form before coming in for your office visit.

Please arrive 10-15 minutes prior to your appointment time to complete registration and a health questionnaire.

When you come for your appointments, please remember to bring the following:

  • Driver’s License or a valid ID
  • Insurance information
  • Referral Letter (if required)
  • Reports, X-rays, MRIs, CT scans, etc. and any other relevant information
  • List of medications (if any)

We recognize that your time is valuable, and we make every effort to stay on schedule. Occasionally emergencies or patients require a little more time and may cause delays beyond our control. We apologize if we keep you waiting.

HOW DO I CANCEL MY APPOINTMENT?

If you need to cancel an appointment, please call the office during business hours at least 1 day prior to your scheduled appointment so that we can offer your appointment time to patients on our waiting list.

HOW DO I ACCESS THE PATIENT PORTAL?

We are pleased to offer a patient portal that gives you access to your health information. Please follow this link to the Patient Portals page, and then choose Physician Office Portal to learn more. To set up your Physician Office Portal for the first time, please contact your physician’s office.

HOW DO I PAY MY BILL?

If you have set up your account, you can pay your bill online through the Healthcare Centers’ Patient Portal. To set up your portal account, please contact your physician office. If you have questions about your Morris Hospital Physician Office bill, please call 815-941-9124.

IS THERE ANYTHING SPECIAL I SHOULD KNOW AS A MEDICARE BENEFICIARY?
