Dr. Mark Passerman, Dwight Healthcare Center of Morris HospitalAs a family medicine physician, Dr. Mark Passerman provides care for the entire family, ages newborn through adulthood, including preventive care, routine check-ups, health risk assessments, immunizations, screening tests, and personalized counseling on maintaining a healthy lifestyle. He is dedicated to treating the whole person and believes in developing long, lasting, caring relationships with his patients and their families.

Dr. Passerman received his medical degree from Des Moines University in Iowa and completed a residency in family medicine at Henry Ford Hospital in Detroit.

When he’s not taking care of patients, he enjoys reading, travel and golf.

ATHAR SAEED, M.D., CARDIOLOGY

Dr. Athar Saeed, CardiologistHaving joined the Morris Hospital Cardiovascular Specialists in 2015, Dr. Athar Saeed came to the Morris Hospital Cardiovascular Specialists following four years of fellowship training in cardiology and interventional cardiology.

His specialized training as an interventional cardiologist makes him qualified to perform advanced testing and procedures in the Morris Hospital Cath Lab, including angiography, angioplasty and stenting of the coronary and vascular arteries. He is board certified in internal medicine, cardiovascular disease and interventional cardiology.

Dr. Saeed believes that education is a cornerstone to the prevention and management of heart disease. That is why he takes time to help patients understand how to manage their condition. Through the use of an evidence-based, quality-directed approach to care, along with effective patient education, Dr. Saeed is committed to partnering with his patients to achieve the common goal of better health, well-being and an improved quality of life.

AMY BOHLAND, F.N.P.-B.C., FAMILY MEDICINE

As a family nurse practitioner, Amy Bohland is qualified to serve as the primary health care provider for patients of all ages. She holds a Family Nurse Practitioner certificate, allowing her to order and interpret diagnostic tests, diagnose disease, provide appropriate treatments and prescribe medications. In her role, Bohland is committed to listening and building trust with her patients to foster an open, honest relationship.

Bohland has worked as a family nurse practitioner since 2019, and she has worked as a registered nurse since 2013. She received her master’s degree in nursing from Governor’s State University. Prior to that, she acquired her bachelor’s degree in nursing from Purdue University Calumet and her associate’s degree in nursing from Joliet Junior College.

KATHY ULIVI, F.N.P.-B.C., FAMILY MEDICINE

Kathy Ulivi, Dwight Healthcare Center of Morris HospitalAs a family nurse practitioner, Kathy Ulivi is qualified to serve as the primary healthcare provider for patients of all ages. She encourages her patients to participate with her in developing their plan of care, encompassing all aspects of physical, emotional and spiritual well being.

Kathy received her Master’s of Nursing Family Nurse Practitioner degree from Olivet Nazarene University. She also holds of Master’s of Nursing in Education from Lewis University. In addition, Kathy is a certified diabetic educator, an oncology certified nurse, and she is board certified in pain management. She also has an interest in integrated behavioral health.

Kathy enjoys spending time with family and friends, teaching, reading and volunteering.

PREPARING FOR YOUR APPOINTMENT

For your convenience, we suggest that you download and complete our Healthcare Centers – General Patient Registration Form before coming in for your office visit.

Please arrive 10-15 minutes prior to your appointment time to complete registration and a health questionnaire.

When you come for your appointments, please remember to bring the following:

  • Driver’s License or a valid ID
  • Insurance information
  • Referral Letter (if required)
  • Reports, X-rays, MRIs, CT scans, etc. and any other relevant information
  • List of medications (if any)

We recognize that your time is valuable, and we make every effort to stay on schedule. Occasionally emergencies or patients require a little more time and may cause delays beyond our control. We apologize if we keep you waiting.

TO CANCEL AN APPOINTMENT

If you need to cancel an appointment, please call the office during business hours at least 1 day prior to your scheduled appointment so that we can offer your appointment time to patients on our waiting list.
